Respect Home Care’s Settling-in program provides transitional care services for older Australians that smooth the process of discharge from the hospital to living back at home. Our transition care services provide temporary home care after surgery or an operation to ensure a smooth, safe recovery.
Whether it’s a planned or unplanned hospital visit, day surgery, or a longer stay, we’re here to help. Our professional and well-trained care team provides personalised support, including pickup from the ward at discharge, transport home, collection of medication, or a pharmacy visit if needed. This is coupled with assistance with basic personal care and a follow-up contact the day after discharge.

Inclusions: • Patient transfer from ward at time of discharge • Aided transportation from hospital to home • Collection of medication or pharmacy needs and fridge basics • Settling you in at home, inclusive of personal care needs, clean bedding, basic meal prep and light domestic care • Next day home care visits/welfare check-in • Up to 4 hours of service. |
Inclusions: • Patient transfer from ward at time of discharge • Aided transportation from hospital to home • Collection of medication or pharmacy needs and fridge basics • Settling you in at home, inclusive of personal care needs, clean bedding, basic meal prep and light domestic care • Next day home care visits/welfare check-in • Up to 2 hours of additional home care service visits within two weeks of discharge focused on personal or domestic assistance • Personalised transportation to post discharge/specialist appointment • Assistance with navigating My Aged Care and assessment process. • Up to 9 hours of service. |

Terms & Conditions:
Services via the Settling-in program are available Monday to Friday. Weekend discharges may be accommodated, strictly subject to availability. Discharge support is available between 7:00 AM and 7:00 PM. Our preferred booking notice is 48 hours although exceptions may be considered based on availability. The program is currently available in areas where Respect Home Care currently operates in Victoria, Western Australia, Tasmania, and New South Wales.
The recipient must be able to safely enter and exit a standard car (wheelchair users welcome). This service does not replace clinical or nursing support. All care is provided by a qualified Home Care Assistant.
The basic package includes up to 30km of travel; the extended package includes up to 50km. Additional kilometres will be charged at $1.50 per kilometre.
Costs associated with medication, mobility aids, equipment, groceries, and other post-hospital essentials are not included and are the responsibility of the care recipient. All pricing is inclusive of GST.
